CONSENT TO THE PROCESSING OF PERSONAL DATA
of users of the websites export67.com, ved.export67.com and the Center's forms in the Yandex Forms service

I, the personal data subject, by completing a form on the website or https://ved.export67.com (hereinafter — the Website) or a form of the Center in the Yandex Forms service and checking the box «I consent to the processing of personal data», freely, of my own will and in my own interest, give my specific, explicit, informed, conscious and unambiguous consent to the processing of my personal data by the Operator: Autonomous Non-Profit Organization «Smolensk Region Export Support Center» (TIN 6732143784, PSRN 1176733007186, KPP 673201001), registered address: 23 Engels Street, Smolensk, Smolensk Region, 214014, Russian Federation; mailing address: 23 Normandie-Niemen Street, 2nd Floor, Smolensk, Smolensk Region, 214030, Russian Federation (hereinafter — the Operator), under the following conditions.

1. Purposes of Processing, List of Personal Data and Duration of Consent
The purpose of processing, the list of data and the duration of consent are determined by the form through which I submit my data:

2. Actions Involving Personal Data and Processing Methods
2.1. I consent to the following actions: collection, recording, systematization, accumulation, storage, clarification (updating, modification), retrieval, use, transfer (provision, access) to the persons specified in Section 3, blocking, deletion and destruction.
2.2. Processing is carried out using a combination of automated and non-automated methods, with data transmitted through the Operator's internal network and the Internet. Recording, systematization, accumulation and storage of my data are carried out in databases located within the territory of the Russian Federation. Cross-border transfer and dissemination of my personal data are not carried out.

3. Persons Processing Personal Data on Behalf of the Operator
I agree that the Operator may entrust the processing of my personal data to the following persons:
–      Yandex LLC (TIN 7736207543, 16 Leo Tolstoy Street, Moscow, 119021, Russian Federation) — the Yandex Metrica web analytics service (purpose 6.6) and the Yandex Forms service (purpose 6.3); data are stored on servers located within the Russian Federation;
–      WebCanape (Twins LLC, TIN: 6731067957) — hosting of the Website and its database, and technical support of the Website.

4. Provision of Data Without Consent
I have been informed that if support is provided to me or to the organization I represent, information about such support will be transferred to the Operator's founder — the Ministry of Economic Development of the Smolensk Region, JSC «Russian Export Center», and the register of support recipients. Such transfer is carried out on the basis of federal law (Clause 2, Part 1, Article 6 of Federal Law No. 152-FZ and Article 8 of Federal Law No. 209-FZ), rather than on the basis of this consent.

5. Procedure for Withdrawing Consent
5.1. I may withdraw my consent at any time by sending a request to the Operator at info@export67.com or by mail to 23 Normandie-Niemen Street, 2nd Floor, Smolensk, Smolensk Region, 214030, Russian Federation. The request must include my last name, first name, patronymic, contact details, a statement withdrawing consent and, if possible, the date and form through which the data were submitted.
5.2. Upon receipt of the withdrawal request, the Operator will cease processing and destroy my personal data within a period not exceeding 30 (thirty) days, except for data whose processing continues on other lawful grounds (Part 2, Article 9 and Part 5, Article 21 of Federal Law No. 152-FZ).

6. Confirmations
By checking the box, I confirm that:
–      I am at least 18 years of age;
–      the information I have provided is accurate;
–      if I provide personal data of other individuals (for example, the head of an organization), I have informed them of the transfer of their data to the Operator and have a lawful basis for such transfer;
–      I have been informed that the Operator's Personal Data Processing Policy is available at /privacy_policy/.
 
The fact that consent has been given (date, time, form and version of the consent text) is recorded electronically by the Operator. The consent checkbox is not selected by default.
